Most practitioners write these from scratch, or open the last patient's letter and edit over it — which is how the wrong name reaches the wrong doctor.
FitDoc builds the summary from the visit notes you already wrote: how many sessions, over what dates, what you did, and how the outcome scores moved from the first visit to the last. You correct it, sign it, and print it on your letterhead.
